Patients arrive in my exam room having heard that bioidentical hormones are natural, plant-based, safer, and custom-made for their body, while "synthetic" hormones are dangerous chemicals pushed by pharmaceutical companies. Almost every clause in that sentence is somewhere between misleading and flatly wrong, and after 20 years in practice, including years in gynecology, I have watched this vocabulary problem cost women money, clarity, and sometimes safety.

Here is the short version up front: "bioidentical" simply means a hormone molecule that is chemically identical to the one your body makes. It is a chemistry description, not a safety rating, not a purity claim, and not a brand. And here is the part the marketing leaves out: many FDA-approved hormone products at your regular pharmacy are bioidentical, while many "custom" compounded products sold as bioidentical are untested, unregulated, and inconsistently dosed.

What "Bioidentical" Actually Means

A bioidentical hormone has the exact same molecular structure as a hormone your body produces. Bioidentical estradiol is, atom for atom, the estradiol your ovaries made; micronized progesterone is structurally identical to your own progesterone. Once absorbed, your body cannot tell them from the ones it manufactured itself.

Now for the details the sales pitch skips:

Bioidentical hormones are made in laboratories. They are synthesized from plant compounds (diosgenin from wild yams, or soy compounds) through multiple chemical steps. No yam contains estradiol, and your body cannot convert yam cream into hormones either; the conversion happens in an industrial lab. "Plant-derived" describes the starting material, not the process.

"Bioidentical" is not a regulated category. Any product can put the word on a label. It tells you about molecular structure only, and only if the manufacturer is being accurate.

Bioidentical does not automatically mean safer. Bioidentical estradiol is still estrogen, with estrogen's benefits and estrogen's risks, including the need for uterine protection in women with a uterus. Same molecule, same biology, same precautions.

What "Synthetic" Actually Means

In common usage, "synthetic hormones" are molecules structurally different from human hormones that activate the same receptors: conjugated equine estrogens (the mix used in the WHI trial) and progestins like medroxyprogesterone acetate, which mimic progesterone's effect on the uterus but are distinct molecules.

Structurally different is not automatically worse; progestins are used deliberately in contraception precisely because their receptor behavior differs usefully. And since bioidentical hormones are also synthesized in labs, the natural-versus-synthetic framing is chemistry theater. The meaningful questions are: what molecule, what dose, what route, and what does the evidence show for that combination?

That said, there are legitimate reasons many clinicians, myself included, often prefer body-identical molecules today. Some evidence, largely observational, suggests micronized progesterone may carry a more favorable breast and clot profile than certain older progestins, and transdermal estradiol avoids the clot risk of oral routes. Those are evidence-based preferences within regulated medicine, not an endorsement of the marketing category "bioidentical."

The Real Dividing Line: FDA-Approved vs. Compounded

Here is the reframe that resolves most of the confusion. The important split in hormone therapy is not bioidentical versus synthetic. It is FDA-approved versus compounded.

FDA-approved products, bioidentical or not, must prove identity, purity, potency, stability, and consistent absorption. Every patch in the box contains what the label says, adverse events are tracked, risks are disclosed, and facilities are inspected.

Compounded products are mixed by individual pharmacies for individual patients. Compounding is legitimate and necessary for genuine needs, like an allergy to an ingredient in every commercial product. But compounded drugs undergo no FDA review for safety, effectiveness, or consistency, and testing has repeatedly found meaningful potency variability between batches and pharmacies.

The compounded "bioidentical hormone replacement therapy" industry, marketed as cBHRT with saliva testing and personalized creams, has grown into a multibillion-dollar business built substantially on the myth that these products are safer and more natural than FDA-approved options. In 2020, the National Academies of Sciences, Engineering, and Medicine reviewed the evidence at the FDA's request and concluded there is insufficient evidence to support the clinical utility of compounded bioidentical hormones, recommending their use be limited to documented allergy or dose-form needs. The highest-level independent review we have says compounded hormones should be the exception, not the default sold at seminars.

FDA-Approved Bioidentical Options You Can Get Today

Here is what surprises my patients most: if you want hormones identical to your own, with regulated quality, you can have both. FDA-approved, body-identical options include:

  • Estradiol transdermal patches — steady delivery, no first-pass liver effect
  • Estradiol gels and sprays — daily skin application, flexible dosing
  • Oral estradiol tablets — inexpensive and widely available
  • Micronized progesterone capsules — body-identical progesterone for uterine protection, often mildly sleep-promoting at bedtime
  • Vaginal estradiol creams, tablets, inserts, and rings — low-dose local treatment for genitourinary symptoms
  • Combination patches pairing estradiol with a progestogen

Every one of these is bioidentical by the marketing industry's own definition, available at ordinary pharmacies, often covered by insurance, and backed by regulated manufacturing. When someone tells you that you must go to a specialty compounding clinic to get "real" bioidentical hormones, they are selling you something that CVS stocks in a regulated form. The Problem with Compounded Hormones

Beyond the absence of FDA review, specific practical problems show up with compounded regimens:

Dose inconsistency. Batch-to-batch potency can drift well outside pharmaceutical tolerances, and with hormones the gap between effective and excessive is not academic.

Unproven combinations. Multiple hormones mixed into one cream, in ratios never studied together in any trial.

Inadequate uterine protection. Progesterone absorbs poorly through skin, so compounded progesterone creams may not protect the uterine lining from estrogen; endometrial overgrowth and cancer cases have been reported. This is my single biggest safety concern with these regimens.

Saliva and dried-urine testing. Salivary hormone levels fluctuate widely, correlate poorly with tissue effects, and are explicitly not recommended by The Menopause Society or the Endocrine Society for dosing. Symptoms and, where relevant, blood levels guide legitimate therapy.

No adverse event tracking, no standardized labeling. Compounded products need not carry the boxed warnings FDA-approved hormones carry, which paradoxically makes some patients believe they are safer. Same molecule, same risks, minus the disclosure.

What About Hormone Pellets?

Pellets, the fastest-growing corner of the compounded market, are implants inserted under the skin every three to six months, releasing estradiol or testosterone continuously. The molecules are bioidentical. The problems are everything else: pellets frequently produce hormone levels far above physiologic ranges, release rates are unpredictable, and once inserted a pellet cannot easily be removed if side effects develop. Studies comparing pellet users to women on FDA-approved therapy found higher rates of abnormal bleeding, mood changes, and acne, alongside those supraphysiologic levels.

There is no FDA-approved estradiol pellet for menopausal hormone therapy. If steady, low-maintenance dosing appeals to you, a weekly patch achieves it with removable, regulated precision.

The Marketing Myths, Dismantled

"Bioidentical hormones are natural; synthetic hormones are chemicals." Both are lab-synthesized chemicals. Bioidentical simply describes the final molecular structure. The yam does not make hormones; the factory does, for both categories.

"Compounded hormones are custom-made for your unique body." Custom ratios sound scientific, but hormone therapy is legitimately individualized by symptoms and response, not by matching a saliva number. There is no evidence custom-mixed ratios outperform titrating a regulated product, and considerable evidence the testing used to justify them is unreliable.

"Bioidentical hormones don't carry the risks of regular HRT." They carry the same estrogen risks, because they are estrogen. The FDA requires identical class warnings on approved bioidentical products. Any clinic claiming their bioidentical hormones have none of the risks is telling you they either do not know the science or hope you do not.

"Doctors won't prescribe bioidenticals because of pharma money." Ordinary providers prescribe FDA-approved bioidentical estradiol and progesterone every day; they are among the most-dispensed menopause products in the country, many available as inexpensive generics. The financial incentive actually runs the other way: cash-pay compounding programs are lucrative precisely because insurance rarely covers them.

What the Medical Societies Actually Say

The alignment here is unusually complete. The Menopause Society recommends FDA-approved hormone therapy, including approved bioidentical formulations, over compounded products, reserving compounding for allergy or unavailable dose forms. ACOG takes the same position and specifically advises against routine salivary testing. The Endocrine Society reached the same conclusions in a scientific statement, and the 2020 National Academies review recommended restricting cBHRT pending evidence that has still not materialized.

When every major professional body lands on the same page, that is not a conspiracy; that is a consensus. And the consensus is not anti-hormone: these same organizations support hormone therapy, frequently with body-identical molecules, for appropriate candidates. The objection is to unregulated products and misleading marketing, not to treating menopause. Common Mistakes I See in My Florida Practice

Choosing a hormone program by its marketing vocabulary. "Natural," "bioidentical," and "custom" are advertising words. The questions that protect you: Is this FDA approved? What molecule and dose? How is my uterus protected? What happens if I get side effects?

Paying cash for compounded creams when a covered equivalent exists. I have seen patients spend $200 to $400 monthly on compounded regimens when an FDA-approved patch plus micronized progesterone, the same molecules, would cost a fraction under insurance.

Using estrogen with progesterone cream for uterine protection. Transdermal progesterone absorbs too poorly to reliably protect the endometrium. If you have a uterus and use systemic estrogen, your progestogen needs a proven route and dose. This mistake has real cancer consequences.

Dosing by saliva or dried-urine tests. No major society recommends these for hormone therapy management. If a clinic's protocol revolves around recurring test kits, that is a revenue model, not medicine.

Accepting supraphysiologic pellet levels as "optimization." Levels far above normal ranges are not optimized; they are excessive, with real side effects and no outcome evidence. Feeling energized at week two does not validate the level at month four.

You deserve better than vocabulary. You deserve the actual questions: which molecule, which dose, which route, what evidence, and what oversight.
