That triple role explains the strange symptom list. Run low, and your red cells become large and inefficient (a megaloblastic anemia), your nerves start misfiring, and your brain chemistry drifts. Unlike most water-soluble vitamins, your liver stores several years' worth of B12, which means deficiency develops slowly and silently, and by the time symptoms appear, the tank has been draining for a long time.
One more essential fact: B12 comes almost exclusively from animal foods (meat, fish, eggs, dairy) and fortified products. Your body also needs stomach acid and a protein called intrinsic factor to absorb it. Anything that interferes with either one can create deficiency even with a perfect diet.

Fatigue is the headline symptom, but it has a particular character: patients describe a heavy, cellular tiredness that sleep doesn't fix. Tingling or numbness in the hands and feet (often described as pins and needles or walking on cotton), burning feet at night, balance problems, and clumsiness all point to nerve involvement. Here's the part I stress with every patient: nerve damage from prolonged B12 deficiency can become permanent. Anemia reverses; neuropathy doesn't always.
Cognitive and mood changes include brain fog, word-finding trouble, poor concentration, irritability, and depression. In older adults, severe deficiency can masquerade as dementia, and it's one of the few reversible causes, which is why checking B12 is standard in any memory workup.
Physical signs include pale or slightly yellow-tinged skin, a sore or unusually smooth red tongue (glossitis), mouth ulcers, shortness of breath with exertion, and heart palpitations from anemia.

People taking metformin
This is the one I most want Florida patients to know. Metformin, the most prescribed diabetes medication in the country, reduces B12 absorption, and long-term use is associated with significantly lower levels. The American Diabetes Association recommends periodic B12 testing for people on long-term metformin, yet most patients on it for years have never had a level drawn. If you take metformin and feel tingling in your feet, don't let anyone call it diabetic neuropathy until B12 has been checked.
People on acid reducers (PPIs and H2 blockers)
Omeprazole, esomeprazole, pantoprazole, famotidine: these medications suppress the stomach acid needed to release B12 from food. Years of daily use, which is extremely common, quietly raises deficiency risk.
Adults over 60
Up to one in five older adults has low or borderline B12, largely because stomach acid production and intrinsic factor decline with age. Absorption fails even when the diet is fine.
Vegans and vegetarians
Plant foods contain essentially no usable B12. Vegans who don't supplement will eventually run out; it's a matter of when, not if. Vegetarians who eat eggs and dairy run lower but still elevated risk.
People with GI conditions or prior GI surgery
Pernicious anemia (an autoimmune loss of intrinsic factor), celiac disease, Crohn's disease, and gastric bypass or sleeve surgery all impair absorption. Bariatric patients need lifelong supplementation and monitoring, full stop.
Why B12 Deficiency Gets Missed So Often
Three reasons. First, B12 isn't in the standard panels; a CBC and metabolic panel can look normal in early deficiency, and the B12 level must be ordered specifically. Second, symptoms are nonspecific and get attributed to age, stress, diabetes, menopause, or depression. Third, deficiency can exist without anemia: roughly a quarter of patients with neurological symptoms have a normal hemoglobin, so a "normal CBC" is falsely reassuring, and folate from fortified foods can mask the anemia while nerve damage continues underneath.

How B12 Testing Works (and Why "Normal" Can Still Be Low)
The starting point is a serum B12 level. Interpretation, per NIH and standard clinical practice:

The borderline zone is where diagnoses get missed. When a level sits in that gray range with suggestive symptoms, we add methylmalonic acid (MMA) and sometimes homocysteine. Oral supplements work well when absorption is intact and even, at high doses, for many absorption problems, because about 1 percent of a large oral dose crosses the gut passively. Typical replacement dosing is 1,000 to 2,000 mcg daily.
B12 injections bypass the gut entirely, which makes them the treatment of choice for pernicious anemia, post-bariatric patients, significant GI disease, and anyone with neurological symptoms where we don't want to gamble on absorption. A common schedule is 1,000 mcg intramuscularly weekly for four weeks, then monthly maintenance. The longer nerves were deprived, the less complete the recovery, which is exactly why early testing matters. Common Mistakes I See in My Florida Practice
Assuming years of "normal labs" ruled it out. B12 is not part of a standard CBC or metabolic panel. Unless someone specifically ordered it, it has never been checked.
Blaming every tingling foot on diabetes. Diabetic neuropathy and B12 neuropathy feel identical, and metformin, the very drug treating the diabetes, depletes B12. I check a level before accepting the diabetic-neuropathy label.
Treating the borderline zone with reassurance. A level of 250 with classic symptoms isn't "fine." That's when MMA testing or a therapeutic trial is warranted.
Supplementing before testing. Starting high-dose B12 before the blood draw muddies the diagnosis, sometimes for months. Test first; it's one tube of blood.
Quitting treatment after feeling better. If the cause is absorption (age, pernicious anemia, bypass surgery, ongoing metformin), the deficiency returns when treatment stops. Maintenance is usually lifelong.
Mega-dosing energy shots instead of finding the cause. A B12 level of 900 doesn't make fatigue disappear if the real culprit is sleep apnea, thyroid disease, or iron deficiency. The goal is diagnosis, not decoration.
