You get the notification: "New lab results available." You open the portal and find three pages of abbreviations, decimals, and reference ranges, with maybe a flag or two in red. If your stomach drops a little at that moment, you're in good company. Learning how to read your blood work shouldn't require a medical degree, yet most patients are handed numbers with no translation.

In two decades of practice, I've walked thousands of patients through these reports line by line, and fifteen minutes of plain-English explanation reliably turns anxiety into understanding. A cholesterol number you understand is a number you can act on.

This guide covers the panels I order most often in primary care: the CBC, the comprehensive metabolic panel, the lipid panel, hemoglobin A1C, TSH, and vitamin D. I'll tell you what each measures, the normal ranges, and what it usually means when a value runs high or low. Bookmark it for your next portal notification.

First, Understand What a "Reference Range" Is

A reference range is the span of values found in roughly 95 percent of healthy people, which means about 5 percent of perfectly healthy people fall slightly outside it on any given test. One mildly flagged value on one blood draw is a data point, not a diagnosis.

What matters far more is pattern and trend: is the value far outside the range, flagged on repeat testing, or moving the wrong direction over time? This is one of the strongest arguments for getting an annual physical with labs: one snapshot tells you little, but five years of snapshots tell a story. Ranges also vary slightly between laboratories, so always compare against the range printed on your own report.

The CBC: Your Blood Cell Report Card

The complete blood count measures the three cell lines your bone marrow produces.

White blood cells (WBC): 4.5 to 11.0 K/µL

These are your immune cells. High usually means your body is fighting something: infection, inflammation, stress, or steroid use. Low can follow viral illnesses or certain medications; persistently low counts deserve follow-up.

Hemoglobin: about 13.5 to 17.5 g/dL for men, 12.0 to 15.5 g/dL for women

Hemoglobin carries oxygen. Low means anemia, which most often traces to iron deficiency, heavy periods, or B12 deficiency. Unexplained anemia, especially over age 45, always gets a workup to rule out slow gastrointestinal blood loss. High can reflect dehydration, smoking, or sleep apnea.

Platelets: 150 to 450 K/µL

Your clotting cells. Mild deviations are common and often transient; big deviations in either direction need attention.

The CBC also includes red cell indices like MCV (average red cell size), which is quietly useful: small cells suggest iron deficiency, large cells point toward B12 or folate deficiency or alcohol effect.

The CMP: Kidneys, Liver, Blood Sugar, and Electrolytes

The comprehensive metabolic panel is 14 tests in one tube, and it's the workhorse of primary care.

Fasting glucose (70 to 99 mg/dL). Between 100 and 125 fasting suggests prediabetes; 126 or higher on two occasions meets criteria for diabetes. This number pairs with your A1C, discussed below.

Creatinine and eGFR. Creatinine is a muscle waste product your kidneys filter; eGFR estimates overall kidney function. Above 60 is generally acceptable, above 90 is normal, and a falling trend matters more than any single value, particularly with diabetes or high blood pressure.

ALT and AST (liver enzymes, roughly 7 to 40 U/L). Elevated liver enzymes are one of the most common flags I see, and in Florida primary care the leading cause by far is fat accumulation in the liver linked to insulin resistance, not alcohol. Medications and viral hepatitis are the other usual suspects.

Electrolytes. Sodium (135 to 145 mEq/L) reflects water balance. Potassium (3.5 to 5.0 mEq/L) matters for heart rhythm and is influenced by blood pressure medications. Calcium (roughly 8.5 to 10.5 mg/dL) mostly reflects parathyroid and vitamin D status.

The Lipid Panel: Your Heart Disease Forecast

Heart disease remains the number one killer of both men and women in America, and the lipid panel is our best routine window into that risk.

  • Total cholesterol: under 200 mg/dL is desirable, but this number alone is the least useful of the four.
  • LDL ("lousy") cholesterol: under 100 mg/dL is optimal for most adults; under 70 is often the goal with established heart disease or diabetes. Treatment decisions hinge on this number.
  • HDL ("healthy") cholesterol: above 40 mg/dL for men and above 50 for women. Higher is generally protective.
  • Triglycerides: under 150 mg/dL. The most lifestyle-responsive number on the panel; it rises with sugar, refined carbs, alcohol, and inactivity, and falls impressively when those change.

High triglycerides with low HDL is also a classic fingerprint of insulin resistance, worth investigating even when glucose still looks normal.

Hemoglobin A1C: Your 3-Month Blood Sugar Average

A1C measures the percentage of your hemoglobin coated with sugar, reflecting your average blood glucose over roughly three months. No fasting required.

  • Below 5.7 percent: normal
  • 5.7 to 6.4 percent: prediabetes
  • 6.5 percent or higher: diabetes

According to the CDC, more than one in three American adults has prediabetes, and over 80 percent don't know it. That statistic plays out in my Brandon exam rooms weekly. The good news: caught early, an elevated A1C is one of the most reversible findings in medicine, and I've written a full guide on how to lower a high A1C safely if this is your flagged number.

TSH: The Thyroid Check

Thyroid-stimulating hormone is the pituitary's thermostat signal to your thyroid, and it moves opposite to thyroid activity. Normal is roughly 0.4 to 4.0 mIU/L.

  • High TSH means an underactive thyroid (hypothyroidism): fatigue, weight gain, cold intolerance, dry skin, constipation, hair thinning.
  • Low TSH means an overactive thyroid (hyperthyroidism): anxiety, palpitations, heat intolerance, unintentional weight loss.

Thyroid problems are dramatically more common in women, and mild ("subclinical") abnormalities are often monitored rather than treated. An abnormal TSH typically gets free T4, and sometimes thyroid antibodies, to complete the picture.

Vitamin D: The Level Everyone Forgets to Order

The 25-hydroxyvitamin D test isn't part of the standard CBC or CMP, which is why deficiency hides so effectively. Below 20 ng/mL is deficient, 20 to 29 is insufficient, and 30 to 50 is my target range. If fatigue, bone aches, or frequent illness are on your list, ask your provider to add it.

Common Mistakes I See in My Florida Practice

Panicking over one mildly flagged value. Reference ranges guarantee that healthy people will occasionally flag. A hemoglobin of 11.9 in a woman is a whisper, not a siren. Trend and context decide what matters.

Ignoring "borderline" results year after year. The opposite error is worse. An A1C of 6.0 or an ALT of 55 that gets shrugged off annually is a missed five-year head start on reversible disease.

Not fasting when the test requires it. A lipid panel and fasting glucose drawn two hours after a Cuban sandwich and sweet tea will mislead everyone. When in doubt, ask whether your draw requires fasting; water and black coffee are fine.

Comparing your numbers to a friend's. Targets are individualized. An LDL of 110 may be acceptable for a healthy 30-year-old and clearly too high for a 60-year-old with diabetes.

Getting labs but never reviewing them with anyone. A portal PDF is not medical care. The value is in the conversation: what the numbers mean together, and what to do next.

Assuming "no news is good news." Portals fail, messages get missed. If you had blood drawn and never heard back, call and close the loop.

Frequently Asked Questions (FAQ)

What blood tests should I get every year?

For most adults, an annual CBC, comprehensive metabolic panel, lipid panel, and A1C form the core, with TSH and vitamin D added based on symptoms and risk factors. Age, sex, family history, weight, and medications personalize the list. These are standard at an annual physical, and most insurance plans cover preventive labs ordered at that visit.

What does it mean if my blood work is flagged high or low?

A flagged value means your result fell outside the lab's reference range, which covers about 95 percent of healthy people, so a mild flag is not automatically a problem. Severity, symptoms, and trend over time determine whether it matters. Far-out-of-range values or repeat abnormals deserve a provider conversation rather than worry or dismissal.

Do I need to fast before blood work?

Fasting for 8 to 12 hours is typically required for a fasting glucose and often recommended for a lipid panel, while a CBC, A1C, TSH, and vitamin D do not require fasting. Water and plain black coffee are generally allowed. If you're unsure, ask when the labs are ordered, because eating beforehand can falsely elevate glucose and triglycerides.

What is a dangerous A1C level?

An A1C of 6.5 percent or higher meets criteria for diabetes, and levels above 9 percent indicate significantly uncontrolled blood sugar with real risk of complications. Between 5.7 and 6.4 percent is prediabetes, which is a warning zone rather than a danger zone. The encouraging part: early elevations respond remarkably well to nutrition, activity, and weight changes.

What is a normal TSH level for a woman?

A normal TSH is roughly 0.4 to 4.0 mIU/L for adults of any sex, though some labs extend the upper limit to about 4.5. High TSH suggests an underactive thyroid and low TSH an overactive one. Because thyroid disease is five to eight times more common in women, I have a low threshold for checking TSH when women report fatigue, weight change, or hair loss.

Why are my liver enzymes high if I don't drink?

Fat accumulation in the liver from insulin resistance is the most common cause of mildly elevated ALT and AST in non-drinkers. Medications, supplements, viral hepatitis, and thyroid problems are other causes. Mild elevations warrant a recheck and often an ultrasound; the prescription is usually weight reduction, less sugar, and more movement rather than any pill.

How often should healthy adults get blood work?

Once a year is a sensible rhythm for most adults, usually timed with an annual physical. Younger adults with pristine results might stretch to every two to three years, while anyone with prediabetes, thyroid disease, abnormal lipids, or ongoing medications needs more frequent monitoring. Trends across years are where blood work earns its keep.

Can I read my own lab results without a doctor?

You can and should read your own results, and guides like this help with the basics, but interpretation still belongs in a conversation with your provider. Numbers interact: a normal glucose means something different beside a high triglyceride and low HDL. Bring questions to your visit; a good clinician welcomes them.

Final Thoughts from a 20-Year FNP

Blood work is the closest thing primary care has to a crystal ball. Long before you feel anything, your labs whisper about the direction your health is heading, and the people who learn to listen are the ones who get to change the ending. I've watched patients reverse prediabetes, normalize liver enzymes, and resolve years of fatigue because a routine panel caught something early. The report is not a verdict. It's a map.
